Promotion Details:

  • $1,000 Deposit for All Surgeries with Dr. Jativa: From May 17th until May 31st, a deposit of $1,000 will be accepted for all surgeries booked during this period. (Applies only to Dr. Jativa)

Promotion Period:

  • This offer is valid for appointments booked and deposits paid between May 17th and May 31st.

Eligibility Criteria:

  • This promotion is open to all new and existing patients who have not yet paid their deposit for a surgery with Dr. Jativa.
  • Eligible patients must meet the BMI (Body Mass Index) criteria of 34 or less and undergo a medical assessment to determine suitability for surgery.

Appointment Scheduling:

  • Appointments are subject to availability and will be scheduled based on Dr. Jativa’s availability.

Deposit and Payment:

  • A non-refundable deposit of $1,000 is required to secure the appointment within the promotion period. The deposit will be deducted from the total cost of the treatment.

Treatment Suitability:

  • The final decision regarding treatment suitability will be made by Dr. Jativa following a thorough medical evaluation.

No Show and Rescheduling Policy:

  • Patients may reschedule their appointment within the clinic’s guidelines, but a fee (amount to be specified) will be charged for no-shows.

Liability Disclaimer:

  • The clinic and Dr. Jativa are not liable for any complications or side effects arising from the treatments. All patients must consent to the procedures after being informed of potential risks.

Patient Responsibilities:

  • Patients are responsible for providing accurate health information and adhering to pre/post-operative instructions.

Modification or Cancellation of Promotion:

  • The clinic reserves the right to modify or cancel the promotion at any time.

Force Majeure:

  • The clinic is not liable for failure to perform obligations due to events beyond its control.

Data Protection:

  • Patient information will be handled in compliance with data protection and privacy laws.

Complaints Procedure:

  • Any complaints should be directed to the clinic’s customer service for resolution.

Final Agreement:

  • These terms constitute the entire agreement between the clinic and the patient regarding the promotion.

Amendment:

  • The promoter reserves the right to amend these terms without prior notice.
